The save I was using was from before I entered the Inn for the first time. Specifically, I saved right on the front steps of the Bannered Mare. Do I need to go further back than that?

No, you don't need to, but it won't hurt if you have another save that's even before Vilja is installed. This is odd. If it doesn't work, can you test her on a new game to make sure that the installation isn't the culprit?

I didn't have any other save files, so I made a new one and tested it. She followed fine. Is there any way to make it work on my old save though? Starting over is a pain, and I'll do it if I have to, but doing the southern holds again when I've done them a million times is getting a bit tedious.

As it is related to problematic save, I don't have any "solutions," but I do have some experimental thoughts you can try. [experimental=potentially dangerous, putting spoiler tags] 1 - Open console, click on Vilja, take note of her RefID 2 - Restart the quest via console (this is risky and not recommended normally): 3 - Fast travel elsewhere 4 - Move Vilja to you by enter this console command: 5 - forces the game evaluate her package with this console command: 6 - Talk to Vilja, and see if she follows. 7 - If she doesn't follow, go to Honningbrew Meadery to see if the thieves are spawned. If so, see if you can retrieve the bottle from them and give it to Vilja. 8 - If you successfully gave the bottle of Vilja, check to see if you are given the reset spell under the restoration school, cast the reset spell on Vilja. See if she follows you then.
If not, then you will have to start a new game to play with Vilja.

Well I hope Vilja will give you a refreshing and enjoyable gameplay so that it's less painful to do another play-through.
