I think Vilja has become frightened of me. Not to say she shouldn't be at this point, but it's getting a little annoying now. In all seriousness; Vilja will not actually follow me. It's as if the "wait here" command is active at all times. When I walk away, she will just stand there and won't make any attempts to follow. When I get far away enough she teleports to me but then she just goes running back to where I left her. She also doesn't want to leave buildings. I'll leave an area and it will take her a good long 15 seconds or so to actually appear outside with me. She also doesn't summon her horse when I get on a horse but that's the least of my problems. It all started around the time I actually got the Cyrodiil Dogwood flute back from the nordic suitor whose name I already forget. Hrolgharaoghr or something. Since he challenged me to the duel at night, around 3 am, I just told Vilja to "wait here" around the corner before I went back and dueled him. Ever since then her following has been wack. I'm not certain though, and I'm really terrified that it might be the cause because I basically don't have a save before then, unless I want to start back at around level 3. Inigo seems to follow me fine if it matters. I've already tried (not in this order): - Reset Vilja command - Quitting and reloading the game - Summon Vilja command when she wasn't there - Giving her a sweetroll
- Splitting up with her and sending her to the Dawnstar bar, then meeting her again. - Leaving her in Dawnstar to learn the loote, then coming back after several days. - Asking nicely - Using the "stop combat, follow me" magic command - Going through the "WARNING" debug dialogue option in the travel options... At least I think it was in the travel options...

You could trya sparring with her. It tends to fix her AI quite a lot.
As a last resort, you could remove all the items you give her, save the game, uninstall the mod, save the game then install again. Follow her quest untill you get the bottle, then I don't recall exactly where's the dialogue, but you need to coment her something is wrong and you already did this (or something like that. I think it's in "travelling together" "you an I" but I'm not sure). This will bring a new NPC to the bannered mare named Beata. You can inform her in which part of Vilja's quest you were. That way you can install the mod again without losing your progress.

Remember the battle royal I had where you had to iron out my script disaster?* Same thing happened to me in Riverwood during the bottle quest. Eventually Vilja froze in her tracks outside Markarth.
If the friendly fight fails it's actually better to revert to an old save or start a new game as that script backlog follows you through a Vilja reboot.
* Ah memories. If I had a nickle for every time I ran the saved game cleaners, chased down orphaned objects... I managed to fix it. 2 years playing to make the mess, 2 solid weeks cleaning it up. Can't remember half the hoops of fire I jumped through. Managed to do it, and that game is still running, handling 21 followers. But I'd start a new game before going through that again.

I was in a hurry and didn't focus propperly in spartan's post. Just get the idea of "something is wrong and can't fix with these methods" and didn't processed what was wrong. Thanks Sniffles!
Certainly this looks like script bloatting. If that's the case, your save is ruined by default. It will only get worse. You could try using save cleaners which may or may not work.
This problem is related to some scripts using functions that call themselves without needing to end them up first, which in case of a momentary lag may end up in a process of incremental self replication (as result of the lag, they replicate before ending, which worsen the lag, which increase the number of replicates before each replicate ends, whic worsen the lag...). Most moders stopped using those functions long ago, but some old mods and specially vanilla game use them.

After a few days of giving Skyrim a general break, I decided to approach it again using your suggestions. The latter mention about script bloating made me queasy, mostly because I did actually have to deal with that problem in New Vegas and I really don't want to repeat that experience. I'm lazy, what can I say?
Luckily, I decided to try the easiest solution first, and just tried amgepo's suggestion of sparring with Vilja. Miraculously; it worked! She seems to be following me just fine now. That being said, I don't know how long that'll last as I've also been playing Skyrim for years now, haven't cleaned a single script, and it's still too early to say if Vilja's following behavior is fixed permanently or not. Infact I dueled her as soon as I started up the game too, so I don't know if giving the game a break a few days reset something.
Due to technical issues I had to wipe every game I own a few weeks ago, which is actually why I started playing Skyrim again with a new character, so I don't know if script bloating is still an issue in this case, or if it's effecting Vilja, or if the dueling even fixed anything.

If it's a near new install script bloating wouldn't be an issue. The friendly fight, what I call relationship reset, often works. Vilja was expecting something or was supposed to do something, but the execution failed or got backlogged. These happen all the time, usually without being noticed. The more major issue with Vilja is if she gets one of these fits, conditions, that blocks her executing a necessary part of her quest. That's when things can snowball. Suggestions: SAVED GAMES! A new saved game with each new session! Deleting a couple hundred redundant saves is a whole lot easier than a chat with Beata. Keep her wardrobe light. Don't use her as a walk in walking closet. I was a major item saving pig and tried using Vilja's wardrobe function. We got trapped in Breezehome and not knowing the console commands I had Vilja pack maximum inventory weight about 20-30 times off to sell at the store before my PC could exit the cell without a game crash. Pay attention to transition cells. Vilja should always exit promptly following the PC. If she doesn't, friendly fight. Fail? Revert to that save you made before you went into the dungeon or ale house of doom. DON'T USE THE QUICKSAVE! Virtual memory and quicksave can turn your hard drive into scrambled eggs. After each of Vilja's quest stages, check to make certain she is acting normally. Best to make a series of saves for the stages. It isn't Vilja's fault as much as certain locations like The Windpeak Inn and the location right outside Eldergleam Sanctuary. Another quest buster is a dragon incoming just as a quest incident or dialogue is supposed to go off. Check your followers for signs of lag after tense situations.
