Ah... Vilja must have delivered the final blow to the dragon. What you desribe sounds exactly like a kill move. I've never seen followers do them, but I don't see why they shouldn't be able to. It's probably just very easy to miss because you don't get the kill-cam slow down like you get when the Dragonborn delivers the final blow.

Otherwise, it must have been a rare accident that she happened to end up where she was fighting. I have no other explanation.

It is indeed a kill-move, and you won't see it on other followers, as we have scripted Vilja to do this (although only very rarely).
